The CARE Report is a system SEU uses to help the Wellness Center team know who needs extra support on campus. Students, faculty, and staff are able to make a CARE Report and have the ability to report anonymously. When making a report, please provide as much information as possible.

For a medical issue: select "Campus Security: Incident Report Form" and indicate "Medical Incident" in the questions section.

For a mental health issue: select "Incident/CARE Reporting Form" and under "Nature of this Report" select "CARE". Please note that the following are considered mental health emergencies and warrant a CARE Report:

  • severe neglect of personal hygiene
  • dramatic change in sleep habits
  • sudden weight gain or loss
  • sudden decline in performance at work or school
  • pronounced changes in mood, such as irritability, uncontrollable anger, anxiety or sadness/crying
  • withdrawal from routine activities and relationships
  • unable to cope with everyday life
  • danger to themselves or others/risky behavior
  • thoughts of suicide
  • attempted suicide
  • substance use/dependence
  • alcohol intoxication
  • violence
  • panic attacks
  • rapid changes in overall behavior
  • making statements not based in reality or false beliefs
  • seeing, hearing, or smelling things that are not real

If your concern does not pertain to one of the following issues listed above, call a SEU counselor for a consultation.

Please note that SEU counselors respond to CARE reports within 24 hours. To honor students' right to privacy, we cannot confirm or deny their attendance or participation in therapy. If you would like to follow up about a CARE report you submitted, we recommend reaching out to the student directly.
